Community Case Management:  Case Managers assist in assessing, implementing, coordinating, monitoring and evaluating the options and services needed to meet the needs of uninsured patients with ongoing medical conditions and/or needs.  Our team consists of an RN who focuses on those with chronic health conditions and a Social Worker who focuses on high ED users including those with chronic pain, substance abuse, and mental health issues.

Patient Navigation:  Our Patient Navigator screens and refers uninsured adults to partner clinics in order to link them to a medical home that best meets their needs.  Education about benefits of having a medical home, referrals for other resources and services, and information about the Affordable Care Act and accessing insurance through the Marketplace are also provided.

Health Insurance Marketplace Navigation: Marketplace Navigators provide education and outreach about insurance coverage options and financial assistance available through the Marketplace.  They provide in-person assistance to answer questions and assist people through the process of enrollment and plan selection.
